Stelkramps¬bakterien producerar efter autolys ett neurotoxiskt protein (tetanospasmin) som i sin struktur liknar botulinustoxinet. Det syntetiseras som en enkelpeptid (150 000 Da). Genom proteolytisk aktivitet övergår denna i en dubbelkedja (A+B-struktur). Endast en antigen typ är beskriven.<br /> Patogenes och patofysiologi<br /> Toxinets B-enhet binder till gangliosider på de motoriska nervcellernas neuron och A-enheten transporteras via axonet till ryggmärgen. Det blockerar liksom botuli-num¬toxinet utsöndringen av acetylkolin men binder till synapser i CNS till skill¬nad från botlinustoxinet som binder perifert. De inhibitoriska impulserna till de motoriska neuronerna resulterar i muskelspasm som kan pågå i veckor. <br /> Symtom och klinisk bild<br /> Stelkramp är ofta associerad med sticksår som initialt kan te sig oskyldiga. Sjukdomen uppträder fr.a. hos ovaccinerade. Patognomont är överkänslighet för ljus och ljud. Den kliniska sjukdomsbilden är dramatisk med muskelspasm och kramper till skillnad från botulism där pareserna är slappa. <br /> Epidemiologi <br /> Bakterien och dess sporer kan isoleras från jord och tarminnehåll hos ett stort antal djurarter.<br /> Under 5-årsperioden 1997 - 2001 rapporterades åtta fall till SMI enligt smitt-skydds¬lagen.<br /> Prevention <br /> Vaccination med tetanustoxoid ingår i det allmänna vaccinationsprogrammet. Boosterdoser ges i samband med djupa förorenade sårskador. <br /> Provtagning och transport<br /> Provtagning är sällan motiverad då den kliniska bilden är typisk. Infektionsfokus kan också vara svårt att identifiera. I övrigt gäller samma rekommendationer som för annan anaerob odling.<br /> Laboratoriediagnostik<br /> Allmänt<br /> Stelkramp är i huvudsak en klinisk diagnos varför frågeställningen är ovanlig på det kliniskt mikrobiologiska laboratoriet. Odling är referensmetodik. Mikroskopi är oftast för okänslig för att påvisa det vanligen låga antalet bakterier. <br /> Referensmetodik:<br /> Referenssubstrat: <br /> Stelkrampsbakterierna växer på blodagar och vanliga an¬aerobsubstrat vilka anses vara referenssubstrat (se bilaga 1).<br /> Isolering<br /> Provmaterialet, oftast från sår, inokuleras på en begränsad del av substratet.
